Transfer from the Hospital to The Springs

• At the time of discharge, orders from your doctor will inform your case worker of your choice for The Springs at Lake Pointe Woods.
• Notification will then be given to The Springs.
• Wheelchair transport from the hospital to The Springs is available.
• The time schedule is determined by the case manager and admissions coordinator.
• Van transportation is also available for out-patient pickup and return home. There is no fee for this service.
